Student Information Updates
To ensure that Columbus State can effectively communicate with and provide support to students, students are encouraged to keep their personal information up-to-date. Students can update or correct information through the following methods:
- Self-Service - Updates to mailing and email addresses, emergency contacts and proxy information, as well Personal Identity Options (Chosen Name, Preferred Pronouns, or Gender Identity) can be completed by the student in Self-Service. Please log into cougarweb.cscc.edu and navigate to "User Options."
- Student Information Update Form - This form is used to process legal name changes, record a student's Selective Service Number, or to correct any errors with a student's legal name, date of birth, or Social Security Number. Please carefully review the form for additional details about the documentation required for each type of update.
- Program of Study Update Form - This form is used to add or change the primary program of study or to add a certificate. Adding a second program of study requires permission from an academic advisor and cannot be processed with this form; please contact your academic advisor if you would like to discuss adding a second program.
How can students submit these forms?
The forms above can be processed in-person, with a picture ID and any required documentation, on the downtown campus at Student Central or on the Delaware campus at Student Services.
Forms may also be submitted electronically to changeinfo@cscc.edu. Please note: Students submitting forms in email must use their CSCC student email for security and identification purposes, and include any additional documents as attachments.
