Procedure for Admission to Nursing Program for Students from R.N. Program 1. All applicants to Nursing must submit a Columbus State application for admissions to the college. 2. All former Columbus State students should check with the Columbus State Student Records and Registration Office to determine if their files need to be reactivated. 3. The applicant will also complete an Application for Nursing Program (Applicant Was Enrolled in a Registered Nurse Program) which is available from the Admission Committee in Nursing. Contact the Nursing Office (287-2506) to speak to with the Admission Committee member responsible for assisting transfer students. 4. The applicant will return the application to the Nursing Admission Committee. Students from an RN program may apply to Nursing during any quarter. 5. The applicant must request an official transcript from the previously attended nursing program and any other related college study be sent to the Columbus State Student Records and Registration Office, Madison Hall, Room 201. This office will also evaluate official college transcripts for possible transfer of applicable non-nursing coursework. Also, send an unofficial transcript to Nursing Admission Committee, Union Hall, Room 507 for review. 6. A minimum grade of “C” or better is required in all Nursing, science, psychology and math courses in the curriculum and for all courses listed in the Admission Requirements. Students accepted to Nursing who do not achieve a minimum grade of “C” or better in any of the following support courses must retake the course(s) prior to the start of their NURS classes or during the quarter in which the course(s) is (are) required in the curriculum plan, in order to remain a student in good standing in the program: BIO 261 (Human Anatomy); BIO 262 (Human Physiology); PSY 240 (Human Growth and Development Through the Life Span); BIO 115 (General Microbiology); BIO 263 (Human Pathophysiology); MATH 135 (Elementary Statistics).

Prospective students are advised of the following policies from the Ohio
Board of Nursing: Section 4723.28 of the Ohio Revised Code, the Law Regulating the Practice of Nursing, states that the Board of Nursing may deny a person the privilege of sitting for the licensing examination related to certain past behaviors or legal history. Each applicant for licensure will be required to answer questions on the application related to whether the individual seeking licensure has been convicted of, found guilty of, pled guilty to, pled no contest to, or received treatment in lieu of conviction for 1) any misdemeanor committed in the course of practice in Ohio, 2) any felony 3) any crime involving gross immorality or moral turpitude 4) any violation of a municipal, county, state, or federal drug law. Any student who may be affected by the above ruling should confer with the Chairperson of Nursing and Related Services upon entry in the program so information about the steps the Board may take before considering an application to sit for the licensing examination can be discussed.
OHIO BOARD OF NURSING LISENSURE APPLICATION REQUIREMENT As of June 2004, the Ohio Board of Nursing requires all applicants for licensure to declare any existing psychiatric condition(s). PROOF OF CITIZENSHIP REQUIRED FOR NCLEX CANDIDATES A new federal law, PRWORA, known as the “Personal Responsibility Act” limits state licensure to U.S. Citizens and other qualified applicants. The State Board of Nursing is required to keep assurance of citizenship on record with applications for licensure. Schools of nursing will provide graduates with the form along with the licensure applications.
8. The Admission Committee will verify: A. The applicant received a minimum grade of “C” or better in all nursing courses completed. B. The applicant has completed at least two quarters/semesters of clinical nursing courses. C. Admission criteria and prerequisite non-nursing courses have been met prior to awarding of transfer credit for Columbus State Nursing course credit. D. The transcript reflects less than 1 year has elapsed since the applicant was last enrolled in and did successfully complete a clinical nursing course.
9. The Admission Committee will then inform the applicant regarding eligibility to proceed with application: A. If the applicant's credentials reflect he/she is "eligible to proceed", the applicant must provide the syllabus and course outline of each nursing course completed to the Admission Committee. B. If the applicant's credentials reflect he/she is "ineligible to proceed", the individual will be informed that he/she may apply to the Nursing through the generic admissions process.
10. The Admission Committee will meet, review and evaluate whether prior nursing course content is comparable to Columbus State Nursing courses. The Admission Committee shall then determine the applicant's eligibility for transfer credit. Transfer credit will only be granted for nursing courses within the first three (3) quarters of Columbus State Nursing curriculum.
