Complete this 7-step process in order to obtain a Preliminary Education Certificate (PEC) from the State Medical Board of Ohio. Upon completion of the process, you will be placed on a list for enrollment eligibility in the Massage Therapy Program.

Step 1
The Columbus State admissions application form is online at https://secure.cscc.edu/application/default.asp. If it has been more than three years since you attended classes at Columbus State, call (614) 287-2453 or 1-800-621-6407, ext. 2453, to reactivate your student file.

Step 2 Complete the Massage Therapy Program Official Application. (.pdf)

Step 3 Written Statement of Interest and Intent. A one page, double spaced, typed personal statement including why you want to complete the Massage Therapy Program, how you plan to use your education, and how you are planning on ensuring your success in this program. Please make sure you sign your statement. This statement of interest and intent must be submitted with your application packet.

Step 4 Even if you have already provided Columbus State Community College with an official high school or GED transcript, an additional official high school or GED transcript MUST be mailed (no faxed copies) directly to the Massage Therapy Program BY THE ISSUING SCHOOL. PLEASE NOTE: Transcripts provided by the prospective student will not be accepted.

Step 5 You must provide a copy of documentation of any and all name changes to Columbus State Community College Massage Therapy Program, examples are a copy of a marriage license or a copy of a divorce decree if name change applies. If you do not have any name changes, please attach a signed statement stating no name changes apply with the application packet.

Step 6 Complete the TOP portion of the Application for Preliminary Education and return with your application packet to the Massage Therapy Program.

PLEASE NOTE: Do not return your completed Preliminary Education application to the State Medical Board of Ohio, as the lower portion needs to be filled out by Columbus State Community College.

Step 7 You must submit a check or money order in the amount of $35.00 made payable to Columbus State Community College with your application packet.

Completed Application Packet must include:

1. Completed and Signed Official Massage Therapy Application
2. Printed and Signed Preliminary Education Form
3. Statement or documentation of name change(s)
4. Non-refundable, non-transferable, Check or Money Order for $35 paid to Columbus State Community College
