To request a replacement diploma you must complete the Official Request for Replacement Diploma form and submit with your payment. The form and the payment may be submitted only by mail or in-person. Faxed copies of the form will not be accepted.
The cost of a replacement diploma is $15 and is delivered via United Parcel Service (UPS). We may accept only forms with a street address as UPS cannot deliver to a PO Box. Please allow 12 weeks after payment has been received for the delivery of your replacement diploma. If you owe money to Columbus State Community College, your diploma will not be released until the debt is paid.
