Student Information Update Form
Student Central can process the Student Information Update Form Form and Program of
Study Update Form. All in-person transactions require a photo ID and some changes
require additional documentation. Forms may also be submitted to changeinfo@cscc.edu.

Program of Study Update Form
The Student Information Update Form is to be used to change the following information on a student's record:
- Contact Information (Address, Phone, Email, Emergency)
- Name Change/Correction
- Social Security Number
- Selective Service Number
- Date of Birth
The Program of Study Update Form is used to change/add the following information on a student's record:
- Program of Study
- Add Certificate Program
- Add a Second Program of Study (with Academic Advisor Approval)
