Share a Concern
To share a concern related to discrimination based on a protected class, related retaliation, and/or sexual harassment/sexual violence, please complete Compliance’s secure online form by clicking the “Share a Concern” button below and a member of the Compliance Office will contact you within two (2) business days.
Please note, this form does not address general concerns or complaints related to conflicts in academic or work environments that are not directly related to discrimination based on a protected class, related retaliation, and/or sexual harassment/sexual violence. General academic concerns should be addressed with an instructor and/or Department Chairperson and general concerns about the work environment should be addressed with a supervisor and/or Human Resources Business Partner. IMPORTANT: Your report will be reviewed during business hours. If you have an emergency, please call 911 or the Columbus State Community College Police Department at (614) 287-2525.
Anonymous Reporting
Students have the option to report anonymously through Compliance’s secure online form; however, the College’s ability to address to an anonymous complaint may be limited. Employees who are designated by College policy as required reporters are not permitted to submit anonymous reports, unless the employee also is the complainant. If an anonymous report includes a crime, it will be included in the College’s crime statistics.
Secure Reporting
The College uses a secure reporting format to address concerns in a prompt and efficient manner. The online report form is sent directly to the Compliance Supervisor/Title IX Coordinator and appropriate Compliance Officer(s). This is the College's preferred method of reporting.
Additional Reporting Methods
You also may share a concern by contacting staff within the Compliance Office via email or phone.
Complaints also can be filed with the Office of Civil Rights (OCR) in the U.S. Department of Education, the Ohio Civil Rights Commission (OCRC) or the Equal Employment Opportunity Commission (EEOC).
