Enrollment History Verification

The Enrollment History Verification Form is used to verify the enrollment status and/or graduation status of the student.

The Enrollment History Verification will be completed only for previous terms or, if currently enrolled, when all fees have been paid.  The Enrollment History Verification Form, or any other form such as an insurance form requesting verification of enrollment should be be completed and submitted to the Records and Registration Department, Madison Hall Room 201, Registration Windows.
