College Transcripts

Please submit official copies of your transcript from each college and/or university that you have attended to Columbus State's Records and Registration Office at the address listed, below.  An official transcript is required of all persons who are seeking transfer credit, or who have completed prerequisite coursework at another institution.

The college representative will ask you to submit your request in writing.  Please print the College Transcript Request Form to make your request.  Please ask the college representative to mail your official transcript to the following address:

When you request an official copy of your college transcript to be sent to Columbus State, you may wish to request an unofficial copy to be sent to you for your use during advising sessions, as needed, when you enroll at the college.  Having an unofficial transcript in hand can be especially helpful if you are applying for admission within a month of the beginning of the academic quarter when you would like to take classes.
