If a Planning Guide was not developed for the 2013-2014 performance year, use the Annual Planning Guide and Performance Review form to summarize the accomplishments of this performance year.

1.Employees complete the Self-Review and submit it to the Reviewing Manager.

2.Reviewing Managers complete the Annual Review and meet in-person with the Employee to discuss it. The Employee should be provided an opportunity to add additional comments once the Annual Review is discussed and before it is submitted to the Administrative Reviewer.

3.The completed form should be printed. Both the Employee and Reviewing Manager need to sign the document. These signatures acknowledge that the Employee and Reviewing Manager have reviewed and discussed the document.

4.Administrative Reviewers approve and sign all Annual Performance Reviews.

5.Full-Time Annual Reviews must be forwarded to Human Resources by July 31 at 4:00 p.m. (Place in sealed envelope and hand-deliver or send via interoffice mail to "HR OFFICE, 115 RHODES HALL.")

Directions for Administrative Reviewers to Complete Annual Reviews
 
1. Read and review Annual Reviews for all employees reporting to the managers you supervise. If you have any questions or concerns address them with the Manager before signing the employee's review.
 
2. Sign the review. Ensure that the Employee and Manager have signed the review. You may want to retain a signed copy for your records (this is not required).
 
3. Your manager/ administrator may want these submitted to him/ her to review.
 
4. Please ensure that all Annual Reviews you sign as Administrative Reviewer are submitted to the Human Resources Department no later than 4:00 p.m. on July 31. (Place in sealed envelope and hand-deliver or send via interoffice mail to "HR OFFICE, 115 RHODES HALL.")
 
5. All Annual Reviews received in the Human Resources Department will be filed in the Employee's Personnel File.
Part Time Employees' Guide

1. The Part-Time Employee Evaluation Form is available on the Faculty & Staff Intranet Forms page.
