If a Planning Guide was not developed for the 2012-2013 performance year, use the Annual Planning Guide and Performance Review form to summarize the accomplishments of this performance year.

1.Employees complete the Self-Review and submit it to the Reviewing Manager.

2.Reviewing Managers complete the Annual Review and meet in-person with the Employee to discuss it. The Employee should be provided an opportunity to add additional comments once the Annual Review is discussed and before it is submitted to the Administrative Reviewer.

3.The completed form should be printed. Both the Employee and Reviewing Manager need to sign the document. These signatures acknowledge that the Employee and Reviewing Manager have reviewed and discussed the document.

4.Administrative Reviewers approve and sign all Annual Performance Reviews.

5.Full-Time Annual Reviews must be forwarded to Human Resources by July 31 at 4:00 p.m.

Part Time Employees' Guide

1. The Part-Time Employee Evaluation Form is available on the Faculty & Staff Intranet Forms page.

2. Reviewing Managers complete the Annual Review and meet in-person with the Employee to discuss it. The Employee should be provided an opportunity to add additional comments once the Annual Review is discussed and before it is submitted to the Administrative Reviewer.

3. The completed form should be printed. Both the Employee and Reviewing Manager need to sign the document. These signatures acknowledge that the Employee and Reviewing Manager have reviewed and discussed the document.
