Underage Change Form - only use this form if you intend to DROP a class or CHANGE sections of a currently scheduled class. Do not use this form to add a new class. You must submit a Course Request Form to add a new class if before the PSEO scheduling deadline. Note: check the CSCC website for add/drop dates.
Instructions for student:
1. Inform your high school guidance counselor and/or your parents/guardians of the change/drop as this could affect your high school eligibility. 2. Complete this form. 3. Sign and date form (see page 2) 4. Fax (614/287-6045), drop-off or mail ONE copy of both pages immediately to have your restrictions lifted. (This form is typically two (2) pages on most printers.)

5. Within 24 business, you will receive an email in your CSCC student email account informing you that your scheduling restrictions have been lifted allowing you to make your drop or section change using one of the following: (a) www.cscc.edu, CougarWeb for Students (b) Call 614/287-5353 (c) Go to Records & Registration or any CSCC Off-Campus Center with the College's Add/Drop form . 6. Any section changes after the course has begun will require you to submit an Add/Drop form with the instructor's signature to Records and Registration (call 614/287-5353 for information). 7. Any drops after the 100% refund period may result in your school/school district requesting reimbursement for tuition, fees and book costs. Any drops after 20% of the quarter has passed will result in a 'W' on your CSCC transcript.

Note: Your restrictions will be lifted within 24 hours/one business day after the form is received in the K-12 Initiatives Office at Columbus State Community College. After restriction is lifted, be advised it is YOUR responsibility to change/drop your class(es). Your restrictions are reinstated after you change/drop your class(es).
